Questionnaire for Service Providers
This research is being conducted by final year Social Science students at the University of New South Wales, under the supervision of Dr. Susan Keen. It is concerned with exploring how the Government reforms of the Disability Day Programs have impacted on families. It is envisaged that the final research project will be presented to DADHC, NCOSS, ACROD and other relevant bodies.
The questionnaire will take approximately 15 minutes to complete. All responses will be confidential and anonymous. Participation is entirely voluntary and you are free to abstain from answering any questions or to withdraw at any time.
